The COACH model is an engagement framework of techniques and tools to use in the care planning process for people experiencing complex health and social issues. COACH trains staff as empowerment coaches, building authentic healing relationships and enabling the co-creation of individualized care plans around a person's long-term goals and also helps create sustainable change. Created by the Camden Coalition in 2014 and codified into a manual in 2016 with the help of the Policylab at the Children's Hospital of Philadelphia, COACH is a framework of techniques and tools designed to help care team members, engage, empower, and build authentic healing relationships with individuals with complex health and social needs. Though originally designed for care management interventions for patients with complex health and social needs, the tools and techniques it describes can be applied to behavior change interventions in a wide range of settings in healthcare, education, social services, and more.
